Job summary

HCA Florida Mercy Hospital is seeking a Social Worker MSW to provide non-clinical, biopsychosocial assessments and interventions on inpatient floors and the ED. You will perform crisis intervention, family engagement, and referrals for financial assistance, focusing on patient-centered care and discharge planning for high-risk patients with psychosocial needs.

Qualifications

• Masters of Social Work (MSW) required.
• 1+ years’ experience or MSW internship required.
• Licensed Clinical Social Worker preferred.
• Critical thinking skills, service excellence and good interpersonal skills.
• Ability to read and comprehend written/verbal instruction PC Skills.

Responsibilities

• Collaborate with RNs, case managers, physicians, insurers, and staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare/Medicaid/private payer regulations and length-of-stay management.
• Perform discharge planning for high-risk patients with psychosocial needs.
• Coordinate meetings between care team, patients, families, and caregivers; support patient/family decisions.
