Job summary

HCA HealthONE Mountain Ridge in Colorado is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions across inpatient floors and the outpatient emergency department, focusing on patient-centered care.

You will collaborate with RN case managers, physicians, and insurance to address discharge barriers, provide crisis intervention, connect patients with resources, and support post-acute placement.

Qualifications

• Master's degree in Social Work is required.
• 1 year of experience or MSW internship.

Responsibilities

• Collaborate with care team to identify and address patient needs.
• Identify and address barriers to discharge and recovery.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations.
• Develop discharge plans for high-risk patients.
• Coordinate meetings between care team and patients/families.
• Provide psychosocial support and counseling for patients and families.
• Offer emotional support during end-of-life and crisis situations.
