Job summary

HCA Florida Osceola Hospital is seeking a Social Worker (MSW) to perform biopsychosocial assessments and interventions on inpatient floors and the ED. The role includes crisis intervention, discharge planning, and connecting patients with financial assistance and resources, with a focus on patient- and family-centered care.

The MSW will work with RN case managers, physicians, and insurers to address barriers to discharge, manage high-risk cases, and support emotional and psychosocial needs

Qualifications

• Bachelor's Degree in Social Work or related field.
• No Travel Required.
• 1 year of experience required.

Responsibilities

• Collaborate with RN case managers, acute care nurses, physicians, and insurance companies to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team, patients, families, and caregivers; assist with decisions respecting preferences and values.
• Provide psychosocial support, counseling, advocacy, and referrals to meet emotional, social, and financial care aspects.
