HCA HealthONE is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions on inpatient floors and the outpatient ED. The role includes crisis intervention, patient/family support, discharge planning, and referrals for financial or resource assistance.

The MSW will work closely with care teams to identify barriers to discharge, coordinate meetings, and support patients and families through complex decisions with compassion and respect.

Qualifications

• MSW degree required.
• 1 year of experience or MSW internship required.
• Strong skills in psychosocial assessment, discharge planning, and patient/family support.

Responsibilities

• Conduct biopsychosocial assessments for inpatients and ED patients.
• Develop discharge plans for high-risk patients and coordinate care meetings.
• Provide emotional support and referrals to financial assistance or resources.
