Job summary

• Required Experience: 1 or more years in Social Work or MSW Internship
• Preferred License: Licensed Clinical Social Worker (LCWS) and Certification in Social Work
• Required Education: Master's degree in Social Work

Responsibilities

• Collaborate with RN case managers, acute care staff, physicians, insurance companies and other staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between the care team, patients, families and caregivers.
