Qualifications

• Master’s Degree in Social Work required.
• Experience in individual, family assessment, crisis intervention, grief counseling, and discharge planning preferred.

Responsabilités

• Conduct psychosocial assessments with patient and family to identify needs and plan post-discharge care.
• Communicate with alternate sites of care and community agencies to address patient needs.
• Document assessments and plans in the EMR reflecting current condition and transition needs.
• Provide consultation and resources to healthcare team.
• Maintain knowledge of community resources and participate in committees or projects as assigned.
