Job summary

Corewell Health in Grand Rapids, MI, seeks a Social Worker for full-time evening shifts (1:00 pm–1:30 am) totaling 36 hours weekly. The role combines pediatric-focused care management with discharge planning and psychosocial assessments.

You will conduct family assessments, coordinate with community resources, and support transitions of care while staying compliant with state regulations. An MSW with 2 years healthcare experience is required; Michigan MSW preferred.

Qualifications

• Master's Degree in Social Work required.
• Minimum 2 years health care related experience post-MSW.
• Preferred: Care management experience.
• Preferred: Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning.
• Preferred: Master Social Worker (MSW) licensed in Michigan.

Responsibilities

• Conduct psychosocial assessments to identify needs and create plans for care transitions.
• Coordinate with alternate sites of care and community resources.
• Provide short-term therapeutic support for patients and families.
• Document assessments and plans in the electronic medical record.
• Collaborate with care teams and participate in relevant committees.
