Corewell Health Wayne Hospital is seeking a Part-time Social Worker to join our interdisciplinary Care Management team. You will assess patients and families with complex psychosocial needs and coordinate discharge planning and community resources.

You will collaborate with the healthcare team, maintain thorough documentation in the EMR, and stay current on regulations affecting care transitions. MSW licensure in Michigan upon hire is required.

Formación

• Must hold a Master of Social Work licensure in Michigan.
• Experience in individual/family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsabilidades

• Conduct psychosocial assessments with patient or family to identify needs and plan.
• Communicate with alternate sites of care and community agencies to address needs.
• Document assessment and plan in EMR reflecting current condition and transition needs.
