Corewell Health is seeking a part-time social worker to join Care Management at Wayne Hospital. You will collaborate with an interdisciplinary team to support patients and families with complex psychosocial needs, including discharge planning and coordination with community resources.

Responsibilities include completing psychosocial assessments, documenting in the EMR, and providing resources to the healthcare team while staying current with federal and state regulations.

Qualifications

• Master's Degree in Social Work and experience in individual/family assessment, crisis intervention, grief and loss counseling, and discharge planning.
• Preferred: MSW/Master of Social Work (State of Michigan).

Responsibilities
