Qualifications

• Required Master’s Degree Social Work.
• Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• Master Social Work LTD - State of Michigan Upon Hire required.

Responsibilities

• Conduct psychosocial assessments with patient and/or family to identify needs and plan accordingly.
• Communicate with alternate sites of care and community agencies to address patient needs.
• Document assessment and plan in electronic medical record reflecting current condition and psychosocial factors.
• Provide consultation and resources to healthcare team members.
• Maintain up-to-date knowledge of community resources.
• Participate in department or regional committees and projects as assigned.
• Develop and maintain knowledge of federal and state regulations as they pertain to role.
