Qualifications

• Minimum one (1) year of clinical social work experience in a health care setting.
• Masters degree in social work (MSW) required.
• Licensed Independent Social Worker (DC) within 6 months of hire OR Licensed Graduate Social Worker (DC) within 6 months of hire
• Licensed Master Social Worker (Maryland) within 6 months of hire
• Licensed Master's Social Worker (Virginia) within 6 months of hire

Responsibilities

• Collaborates with health care team to coordinate cost-effective services aligned with the member's plan of care.
• Assist members in achieving optimal independence and quality of life.
• Coordinate and schedule family meetings and provide counseling and education as needed.
• Identify and facilitate access to local, state, and federally funded programs (e.g., Medicaid, Medicare).
• Document case management activities consistently and in compliance with standards.
• Develop and monitor a plan of care with the nurse case manager and team.
• Provide support to members, families and caregivers, including end-of-life issues and directives.
• Advocate for appropriate resources and community supports.
• Adhere to NASW Code of Ethics and maintain current knowledge of relevant laws and policies.
