Job summary

Qualifications

• Minimum one (1) year of clinical social work experience in a health care setting.
• Masters degree in social work (MSW) required.
• Credentials from multiple states: DC licensure within 6 months of hire, Maryland LMSW within 6 months, Virginia LMSW within 6 months.

Responsibilities

• Collaborate with the health care team to coordinate cost-effective services aligned with the member’s plan of care.
• Complete psychosocial assessments evaluating goals, support, health status, and resources to reduce inappropriate utilization of medical services.
• Develop and monitor a plan of care with nurse case manager to promote optimal functioning and quality of life.
• Identify and facilitate appropriate organizational and community resources for the plan of care in a cost-effective, timely manner.
• Document case management activities consistently in accordance with standards.
