Qualifications

• Minimum one year of clinical social work experience in a health care setting.
• Master's degree in social work (MSW) required.
• Credentials from multiple states required on hire; DC, MD, and VA licensure expectations.

Responsibilities

• Coordinate and manage an assigned caseload to meet care goals and optimize resource use.
• Perform comprehensive psychosocial assessments covering goals, support, status, and environment.
• Develop and monitor a care plan to promote functioning and quality of life.
• Identify and facilitate access to local, state, and federal programs (Medicaid, Medicare).
• Document case management activities consistently in accordance with standards.
• Provide counseling and education to members, families, and care team.
• Schedule and coordinate family meetings and guardianship paperwork as needed.
• Ensure compliance with NASW Code of Ethics and evolving local regulations.
