Job summary

Qualifications

• Minimum one (1) year of clinical social work experience in a health care setting.
• Master's degree in social work (MSW) required.
• Credentials from multiple states; LICSW/LCSW-type credentials required or pending as specified.

Responsibilities

• Coordinate with health care team to facilitate cost-effective services aligned with the care plan.
• Maintain documentation of case management activities in compliance with standards.
