Job summary

Qualifications

• Minimum one year of clinical social work in a health care setting.
• MSW degree required.

Responsibilities

• Collaborate with the health care team to coordinate cost-effective services.
• Conduct psychosocial assessments and help plan care.
• Coordinate with family, community resources, and care settings.
• Document case management activities per standards and compliance.
• Facilitate access to Medicaid/Medicare and other programs.
• Provide supportive counseling and education to members and families.
• Maintain knowledge of laws and NASW ethics in practice.
• Schedule family meetings and complete guardianship paperwork as needed.
• Inpatient only: UAIS forms for Virginia Medicaid as required.
