Responsibilities

• Collaborate with care team to coordinate cost-effective services aligned with the plan of care.
• Complete psychosocial assessments evaluating goals, support, resources, and status to reduce unnecessary hospital utilization.
• Develop and monitor a plan of care with the nurse case manager to improve functioning and quality of life.
• Identify and connect members to organizational and community resources.
• Document case management activities consistently per standards.
