Job summary

Kaiser Permanente in Falls Church, VA seeks a Social Worker Case Manager to coordinate patient-centered care across the health care team, helping members achieve independence and quality of life.

Requires a Master of Social Work and state licenses; 2+ years in case management or 3+ years clinical experience; strong communication and EMR skills; in-person assessments in home and community settings are expected.

Qualifications

• MSW required.
• Minimum two (2) years of experience in case management or three (3) years of clinical experience.
• Virginia Medicaid: experience with Cardinal Care Priority Population criteria is preferred.

Responsibilities

• Collaborate with the health care team to assess health status, functional limitations, psychological status, social support systems, resources, environmental factors, and response to treatment.
• Use motivational interviewing to assess readiness for change and create care plans.
• Provide counseling and education to members, families, caregivers and health care teams, including end-of-life issues and Advance Directives.
• Document case management activities accurately and in compliance with standards.
• Facilitate access to local, state, and federal programs (e.g., Medicaid, Medicare, Disability) or refer to community resources.
• Coordinate care across the care continuum for behavioral health and substance abuse services.
• Identify problems, perform psychosocial assessments, provide financial counseling/referral, and assist with health-related legal processes.
