Job summary

Kaiser Permanente in Maryland seeks a licensed Master Social Worker (MSW) to join our patient-centered care team. You will coordinate services, assess needs, provide counseling, and connect members with community resources to achieve optimal independence and quality of life.

Under supervision of the care team, you will develop and monitor care plans, facilitate placement, and support families through programs such as Medicaid/Medicare referrals while maintaining NASW standards.

Qualifications

• Minimum one (1) year of clinical social work experience in a health care setting required.
• Masters degree in social work (MSW) required.
• Credentials from multiple states required at hire; additional post-hire credentials may be required.

Responsibilities

• Collaborate with health care team to coordinate cost-effective services.
• Complete psychosocial assessments to evaluate goals, resources and status.
• Develop and monitor care plans to promote optimal functioning and quality of life.
• Advocate and connect members to community resources and programs.
• Document case management activities in compliance with standards.
• Analyze outcomes to improve program quality and cost effectiveness.
• Facilitate access to local/state/federal programs (Medicaid/Medicare).
• Provide supportive counseling and education to members and families.
