You will assess patient needs, educate families, coordinate with insurance and community resources, and contribute to quality improvement and patient satisfaction initiatives.

Qualifications

• Master's degree in Social Work required.
• 0–2+ years related experience is common for entry level roles.
• Ability to provide psychosocial services and counseling to patients and families.

Responsibilities

• Assess patients' psychosocial status and plan care in collaboration with the team.
• Provide supportive counseling to patients and families within licensure scope.
• Educate patients and families about resources, transplant options, and financial assistance.
