Job summary

Fresenius Medical Care in Minnesota is seeking an entry-level Medical Social Worker (MSW) to provide psychosocial services to patients undergoing in-center or home dialysis where applicable. You will work within the interdisciplinary team to promote adjustment and quality of life, educate staff on psychosocial impact, and support transplant referrals as needed.

The role requires a Master's in Social Work and state licensure, with 0–2+ years of related experience.

Qualifications

• Masters in Social Work required and licensed to practice.
• 0–2+ years of related experience.
• Knowledge of dialysis patient care helpful.

Responsibilities

• Assess psychosocial status and plan care as part of the interdisciplinary team.
• Provide counseling and education to patients and families.
• Coordinate referrals to community resources.
• Participate in quality improvement activities and documentation.
• Maintain compliance with FMCNA Social Work policy.
