Job summary

Fresenius Medical Care seeks a skilled Social Worker to support patients on in-center and home dialysis when applicable. You will assess psychosocial status, provide counseling, educate patients and families, and coordinate with an interdisciplinary team to optimize rehabilitation and quality of life.

Responsibilities include care planning, transplant referral assistance, and connecting patients to community resources while adhering to FMCNA policies and quality goals.

Qualifications

• Masters in Social Work and state license required.
• 2–5 years related social work experience preferred.
• Knowledge of ESRD psychosocial issues and transplant processes.

Responsibilities
