Job summary

Fresenius Medical Care is seeking an entry-level Master’s in Social Work to provide psychosocial services for in-center and home dialysis patients. You will work with the multidisciplinary team to support rehabilitation, adjust to dialysis, and improve quality of life.

You will assess psychosocial needs, deliver counseling, educate patients and staff, and facilitate transplant referrals, while ensuring documentation and adherence to policies and quality goals.

Qualifications

• Masters in Social Work required.
• 0–2 years of related experience preferred (entry level).
• Must have state required license to practice.
• Meets licensure requirements in the state of employment.

Responsibilities

• Assess patients' psychosocial status and collaborate with the healthcare team to identify interventions.
• Provide counseling to patients and families to support rehabilitation and quality of life.
• Educate staff on psychosocial aspects of Chronic Kidney Disease and treatment options.
• Assist with transplant referrals and waitlist management.
• Coordinate with community resources for financial and support services.
• Document MSW interactions and interventions in patient records.
