Qualifications

• Masters in Social Work required.
• Must have state-required license.
• Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed.

Responsibilities

• Assess patients' psychosocial status, strengths and needs as part of comprehensive patient assessment.
• Participate in care planning with patient and healthcare team to identify interventions to improve rehabilitation and quality of life.
• Provide monitoring, counseling, and education to patients considering transplant and psychosocial issues related to ESRD.
• Refer patients to community resources and assist with access to vocational rehabilitation or education.
• Document MSW interactions and interventions as required by policy and state licensure.
• Support quality indicators reviews, adherence, and patient education regarding treatment modalities and advance directives.
