Job summary

Fresenius Medical Care is seeking an entry-level Master’s in Social Work to provide psychosocial services for dialysis patients in our Webster, MA facility. The role collaborates with the interdisciplinary team to assess needs, plan interventions, and support patient adjustment and quality of life.

Responsibilities include counseling, education for patients and families, and referrals to community resources.

Qualifications

• Masters in Social Work required and state license required.
• Experience range 0–2 years in social work or healthcare settings.
• Knowledge of dialysis patient needs and end-stage renal disease is a plus.
• Ability to educate patients and families effectively.

Responsibilities

• Assess psychosocial status and plan patient-centered interventions.
• Collaborate with healthcare team on rehabilitation goals and outcomes.
• Provide counseling and support for transplant discussions.
• Refer patients to community resources and vocational services.
• Assist with advance directives discussions and family education.
• Support quality improvement activities and patient education initiatives.
