Job summary

Fresenius Medical Care is seeking an entry-level Medical Social Worker to provide psychosocial services for patients undergoing in-center or home dialysis. Qualifications

• Masters in Social Work required.
• State license required to practice social work.

Responsibilities

• Assess patients' psychosocial status and develop care plans.
• Provide counseling and support to patients and families.
• Collaborate with the interdisciplinary team to support rehabilitation and QoL.
• Educate staff on emotional, psychological, and behavioral impact of CKD.
• Refer patients to community resources and vocational rehab as needed.
• Support transplant referral process and waitlist management.
• Document MSW interactions and interventions per policy.
• Participate in quality improvement activities and patient education.
