Job summary

Fresenius Medical Care is seeking an entry-level MSW to provide psychosocial services to in-center and home dialysis patients, supporting adjustment and quality of life as part of the care team.

You will assess psychosocial needs, participate in care planning, offer counseling, educate patients and families, and refer to community resources to facilitate rehabilitation and continued treatment. You will document interactions per policy and contribute to quality improvement efforts.

Qualifications

• Masters of Social Work required.
• 0–2 years of related experience preferred.
• Ability to work with multidisciplinary teams and collaborate with medical staff.

Responsibilities

• Assess patients' psychosocial status and needs within the care team.
• Develop and monitor care plans to support rehabilitation and quality of life.
• Provide counseling and education to patients and families.
• Coordinate referrals to community resources and services.
