Job summary

Qualifications

• Master's in Social Work and state licensure required.
• 0–2 years of related social work experience.
• Excellent communication and counseling skills.

Responsibilities

• Assess psychosocial status and plan care in collaboration with the patient and team.
• Provide counseling to patients and families to support adjustment to dialysis.
• Educate patients on treatment options including transplant referrals.
• Coordinate referrals to community resources and vocational rehabilitation.
• Support quality indicators and participate in monthly team meetings.
• Adhere to FMCNA policies and maintain accurate documentation.
