Job summary

PowerBack Rehabilitation is seeking a licensed Speech-Language Pathologist to deliver outpatient therapy in home and facility settings in Westfield, MA. The role emphasizes mobile, patient-centered care with flexible scheduling and a salaried position.

You will evaluate and treat adults in homes, independent living, and assisted living facilities, coordinating with the rehab team and physicians to restore communication and swallowing functions and maintaining detailed documentation for care

Qualifications

• Must be licensed as a Speech-Language Pathologist in the state(s) of practice or eligible for licensure.
• Must have reliable transportation to patients’ residences.

Responsibilities

• Provide outpatient speech-language pathology services in home-based and facility settings.
• Coordinate care with rehabilitation team and physicians to tailor therapy plans.
• Document assessments, progress notes and plans of care in a timely manner.
• Supervise CFYs, students and support personnel per licensure requirements.
• Order supplies, maintain equipment, and ensure a safe, supportive home environment.
