Qualifications

• Must hold a Speech Language Pathologist license in the state(s) of practice, or be eligible for licensure.
• Must have a valid driver’s license or reliable transportation to travel to patients’ homes.

Responsibilities

• Provide outpatient speech-language pathology services in the patient’s home or community setting.
• Assess, diagnose and treat communication and swallowing disorders.
• Develop and implement individualized treatment plans and goals.
• Coordinate with care teams and caregivers to ensure continuity of care.
• Document assessments, progress notes and billing information in a timely manner.
