Powerback Rehabilitation seeks a Physical Therapist to deliver in-home outpatient rehabilitation to older adults across Vernon, Ellington, Tolland, South Windsor, CT. The role emphasizes comprehensive evaluation, tailored therapy plans, and caregiver education, with a flexible schedule and home-based practice.

The ideal candidate holds a CT PT license, a Master’s in Physical Therapy, and experience in outpatient or home care.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or equivalent required.
• Experience in outpatient/home-based rehab preferred.

Responsibilities

• Provide in-home outpatient rehabilitation for older adults.
• Develop and implement individualized therapy plans.
• Educate patients and caregivers on exercises and safety.
• Coordinate with the rehab team and other disciplines as needed.
• Maintain documentation and schedule in a timely manner.
• Supervise assistants and students per licensure rules.
• Ensure safety and proper equipment maintenance.
