Job summary

Powerback Rehabilitation To You is seeking a Physical Therapist to join our mobile outpatient therapy team serving Vernon, Ellington, and surrounding CT communities. The role focuses on in-home outpatient rehabilitation for older adults, delivering personalized therapy in the patient's home or in independent living settings.

You will assess, plan, and implement PT programs, collaborate with the rehab team, educate patients and caregivers, and adapt treatment to promote independence.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy required.
• In-home/outpatient PT experience preferred.
• Excellent communication and patient education skills.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Develop and deliver individualized therapy programs and adjust as needed.
• Collaborate with rehabilitation team and other disciplines.
