Powerback Rehabilitation seeks a Physical Therapist to provide mobile outpatient rehabilitation in Watertown, CT, delivering in-home PT services to older adults in private homes or independent living facilities.

Qualifications

• Must hold a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Master’s in PT plus a Bachelor’s in PT; or 5 years of PT experience.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Assess needs, develop and deliver PT programs; refer to other disciplines as needed.
• Educate patients and caregivers to promote independence and safety.
• Supervise PTA, temporary licensees, students, and support personnel per licensure.
• Adhere to treatment schedule and maintain timely clinical and billing documentation.
• Promote a safe environment and manage equipment and supplies.
