Powerback Rehabilitation to You seeks a Physical Therapist for PRN mobile outpatient care in Suffield, CT and Agawam, MA. The role delivers in-home outpatient rehabilitation to older adults with a flexible schedule, emphasizing patient-centered care and functional independence.

You will evaluate, treat, and educate patients, coordinating with the rehab team to address individual needs. A valid PT license is required, with opportunities for professional development and travel across the U.S.

Qualifications

• Must have a Physical Therapy license in the state(s) of practice.
• Must have a Master’s degree in Physical Therapy or equivalent.
• Bachelor’s degree in Physical Therapy with 5 years PT experience also acceptable.
• Must have a valid driver's license or reliable transportation.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Evaluate and treat patients, develop and deliver PT programs.
• Educate patients and caregivers to promote independence.
• Coordinate with other rehabilitation team members for comprehensive care.
• Maintain documentation and ensure timely billing and scheduling.
