Job summary

Powerback Rehabilitation To You seeks a Physical Therapist Assistant for mobile outpatient care across Newington and Southington, CT. The role covers ALF/ILF and community homes with a flexible schedule and a patient-centered approach to rehab.

You will deliver in-home physical therapy under supervision, document progress, and support functional independence in older adults while collaborating with a licensed PT and care team.

Qualifications

• Must have an active license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in patients' homes or in assisted living settings.
• Follow treatment plans under a licensed physical therapist and document care.
• Educate patients and caregivers on exercises and safety as part of home programs.
• Maintain equipment, scheduling, and clinical/billing documentation.
