Job summary

Powerback Rehabilitation is seeking a Physical Therapist PRN (10 hrs/wk) in West Hartford, CT. The role focuses on outpatient rehabilitation and in-home visits for older adults, with flexible scheduling and a supportive clinical mentorship environment.

You will assess, design, and implement PT plans, collaborate with the rehab team, and guide patients toward independent functional goals in home and community settings.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent combo as described.
• Bachelor's in Physical Therapy with 5 years of PT experience is acceptable.
• Foreign PT degree must be evaluated for US equivalence.
• Must have a valid driver's license or reliable transportation.

Responsibilities

• Provide outpatient rehabilitation services in home-based settings for older adults.
• Develop and deliver individualized physical therapy programs, coordinating referrals as needed.
• Educate patients and caregivers to promote independence and safe home exercise.
• Supervise PT assistants, temporary licensees, students per licensure requirements.
• Document clinical and billing information; attend meetings as required.

Education

Licensed Physical Therapist in the state(s) of practice

Master's degree in Physical Therapy

Master's in PT-related field + Bachelor's in PT

Bachelor's degree in Physical Therapy + 5 years PT experience

Foreign PT degree evaluated for US equivalence

Valid driver's license / reliable transportation
