Qualifications

• Must hold a Physical Therapy license in the practice states.
• Master's degree in Physical Therapy or equivalent.
• Experience in outpatient or home-based rehabilitation is a plus.

Responsibilities

• Provide outpatient rehabilitation in in-home settings for older adults.
• Assess, develop, and deliver individualized PT programs.
• Collaborate with rehab team to address comprehensive patient needs.
• Educate patients and caregivers to promote independence.
