Job summary
21st Century Home Health Services is seeking a Part Time Physical Therapist Case Manager to provide in-home evaluations and develop care plans. You will work with PTAs, help patients regain mobility, and coordinate with physicians to ensure high-quality care.
This role emphasizes autonomy, collaboration, and community-specific service in the Vallejo area. Requirements include a CA license, BLS/CPR, and a CA driver’s license, with home health experience and team-oriented skills highly valued.
Qualifications
- Current unrestricted CA Physical Therapist License.
- Current BLS/CPR Certification.
- Valid CA Driver's License.
- Knowledge of local community resources is required.
- Experience in home health care preferred.
- Experience with high acuity patients is a plus.
- Experience working with an interdisciplinary team is highly desired.
- Case management fundamentals is highly desired.
- Self-directed, empathetic, and strong interpersonal skills.
Responsibilities
- Perform in-home evaluations and develop care plans with PTAs to follow.
- Help patients with mobility and regain independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure quality care.
- Evaluate and report patients’ progress to MD.
