Job summary

21st Century Home Health Services in California is seeking a Part Time/Weekend Physical Therapist Case Manager to assess patients in their homes, develop care plans, and coordinate with Physicians and PTAs.

You will evaluate and treat to restore function, support independence, and communicate progress. Requirements include a CA Physical Therapist license, BLS/CPR, a valid CA driver's license, and experience in home health.

Qualifications

• CA Physical Therapist license required and current in good standing.
• BLS/CPR certification is mandatory.
• Experience in home health care highly preferred.

Responsibilities

• Perform home evaluations and create care plans with PTA follow-up.
• Help patients regain mobility and independence.
• Create treatment plans and coordinate with physicians.
• Document progress and report to MD.
