Job summary

West Coast Wound & Skin Care is seeking a skilled Physical Therapist Assistant (PTA) to deliver home-based physical therapy across the Fresno region. Work under a licensed Physical Therapist, implementing established plans of care, educating patients and caregivers, and monitoring progress in a mobile setting.

Ideal candidates are autonomous, reliable, and able to manage a caseload while collaborating with a supervising PT.

Qualifications

• Current and valid PTA license in good standing
• Graduate of an accredited PTA program
• Strong communication and interpersonal skills
• Ability to work independently in a home-based setting
• Valid driver’s license and reliable transportation

Responsibilities

• Travel to patients' homes to provide physical therapy under supervision
• Review plan of care and treatment goals with supervising Physical Therapist
• Provide skilled interventions per plan of care (exercises, gait training, balance, manual therapy as appropriate)
• Educate patients and caregivers on exercises, safety, and home program adherence
• Document treatment and patient progress accurately and timely
• Maintain compliance with regulations and payer requirements
• Communicate changes in patient condition to supervising PT
