West Coast Wound & Skin Care seeks a dedicated Physical Therapist to deliver high-quality home-based physical therapy throughout the Antelope Valley region. You will evaluate patients in their homes, develop individualized plans of care, and provide skilled interventions to improve mobility and safety.

Qualifikationen

• Current and valid California Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.
• Strong clinical reasoning and patient assessment skills.
• Ability to independently perform comprehensive PT evaluations and develop appropriate plans of care.

Aufgaben

• Perform home-based PT evaluations and develop plans of care.
• Provide therapeutic exercises, gait and balance training, and manual therapy when appropriate.
• Educate patients and caregivers on mobility safety and home exercise programs.
• Document evaluations, progress notes, and discharge summaries.
