Job summary

Powerback Rehabilitation To You is hiring a Physical Therapist to deliver in-home outpatient rehabilitation for older adults across Dover, NH and surrounding towns. The role supports a flexible schedule with part-time or full-time hours based on patient needs and clinician location.

The therapist will evaluate and treat in the patient’s home, coordinate with the broader rehabilitation team, and provide education to patients and caregivers to maximize independence and safety.

Qualifications

• Master’s degree in Physical Therapy or equivalent.
• Bachelor’s degree in Physical Therapy with 5+ years of progressive PT experience (if applicable).
• Valid Physical Therapy license in state(s) of practice.

Responsibilities

• Provide outpatient rehabilitation services in home-based settings for older adults.
• Assess, develop, and deliver individualized physical therapy programs.
• Coordinate referrals with other disciplines to address comprehensive patient needs.
• Educate patients and caregivers to promote independence and safety at home.
• Supervise PTAs and students as permitted by licensure and state requirements.
• Maintain documentation and contribute to care planning and scheduling.
