Powerback Rehabilitation in Concord, NH is seeking a part-time Physical Therapist for home-based outpatient therapy. You will deliver individualized rehabilitation in patients' homes, assessing needs and developing care plans with families involved.

You will participate in a mentorship program for new grads, access free CEUs, and benefit from a flexible schedule as part of a nationwide in-home care model. This role emphasizes patient-centered recovery.

Competenze

• Must have a valid Physical Therapy license.
• Master's degree in Physical Therapy is required.
• Experience in home-based rehabilitation is preferred.

Mansioni

• Provide outpatient rehabilitation in patients' homes or in independent living settings.
• Develop and adjust individualized treatment plans with patient and caregiver input.
• Assess patient progress and document outcomes and services.
• Educate patients and caregivers on exercises, safety, and home programs.
• Coordinate with the rehabilitation team to address patient needs.
