Qualifications

• Must hold a valid Physical Therapy license in the practicing state(s).
• Master’s degree in Physical Therapy or equivalent required.
• Experience in outpatient rehab and home-based care preferred.

Responsibilities

• Provide outpatient rehabilitation services in in-home and facility settings.
• Develop and implement individualized therapy plans to restore function.
• Coordinate with the rehab team and other disciplines for comprehensive care.
• Educate patients and caregivers to promote independence and safety.
• Supervise PT Assistants and students as applicable.
• Maintain clinical and billing documentation in a timely manner.

Education

Master’s degree in Physical Therapy

Bachelor’s degree in Physical Therapy + 5 years of experience
