PowerBack Rehabilitation is seeking a Physical Therapist for full-time Mobile Outpatient Therapy in Nashua, NH. The therapist will evaluate and treat older adults in-home or in community settings, delivering personalized rehab plans to promote independence and safe mobility.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice.
• Master’s degree in Physical Therapy or equivalent required.
• Bachelor’s degree in Physical Therapy with significant experience acceptable.
• Driver’s license and reliable transportation to patient homes.

Responsibilities

• Provide outpatient rehab in home and community settings for older adults.
• Develop and deliver individualized therapy plans and educate patients and caregivers.
• Supervise PT assistants and students as permitted by state licensure.
• Collaborate with rehab team to address comprehensive patient needs.
• Maintain timely documentation and scheduling, ensuring safety.
