Job summary

PowerBack Rehabilitation is seeking a licensed Physical Therapist to deliver high-quality outpatient rehabilitation in the patient’s home or in housing facility settings. You’ll assess, design, and supervise personalized therapy programs for older adults, educate patients and families, and coordinate with a multidisciplinary team.

Qualifications

• Must hold a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy or an equivalent combination of degrees.
• Bachelor’s degree in Physical Therapy with 5 years of progressive PT experience (alternative path).

Responsibilities

• Provide outpatient rehabilitation services in the home setting for older adults.
• Develop, implement, and adjust individualized physical therapy plans.
• Educate patients and caregivers on exercises and home programs.
• Supervise PT assistants and support staff as applicable.
• Ensure timely documentation and coordination with care teams.
• Promote safety and adherence to clinical standards.
