Job summary

Powerback Rehabilitation, LLC is seeking a licensed Physical Therapist to provide in-home outpatient therapy in Hanover, NH. The role focuses on evaluating and treating older adults in their homes or independent living facilities to restore function and independence.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice.
• Experience in outpatient/home health PT is preferred.
• Ability to educate patients and caregivers.

Responsibilities

• Provide outpatient rehabilitation in patient homes or ILFs.
• Develop and deliver individualized therapy plans.
• Supervise assistants and students as needed.
• Document assessments, progress, and outcomes.
• Promote safety and infection control in home settings.
