Job summary

PowerBack Rehabilitation is seeking a Physical Therapist for full-time in Greenfield, MA, delivering in-home outpatient therapy. The role emphasizes evaluation and treatment in the patient's home or in independent living facilities, with flexible scheduling and a focus on meaningful impact.

The ideal candidate holds a Master's in Physical Therapy (or equivalent), a valid PT license, and can transport to patient homes.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
• Master's degree in Physical Therapy (or equivalent with Bachelor’s in PT).
• Driver’s license or reliable transportation to patient homes.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings or in living facilities.
• Develop and deliver individualized therapy programs to restore function.
• Educate patients and caregivers on home exercise and safety.
• Supervise PT Assistants, temporary licensees, students when applicable.
• Document care and progress in accordance with state and regulatory requirements.
• Promote health and mobility in home and community settings.
