Job summary

Powerback Rehabilitation is seeking a Physical Therapist to provide outpatient rehabilitation services in-home for older adults across Vernon, CT and surrounding areas. The role focuses on evaluating needs, designing personalized therapy programs, and coordinating with the rehab team to address comprehensive patient goals.

Qualifications

• Must hold a valid Physical Therapy license in applicable state(s).
• Master’s degree in Physical Therapy required; another PT-related Master’s/Bachelor’s with experience is acceptable.
• Experience providing outpatient PT services, including in-home settings, is preferred.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Develop and deliver individualized physical therapy programs.
• Collaborate with rehab team and refer to other disciplines as needed.
• Supervise PT assistants and students per licensure.
• Document care and maintain patient records; ensure home safety and caregiver training.
