Job summary

PowerBack Rehabilitation is seeking a licensed Physical Therapist for a full-time mobile outpatient role across Vernon, Ellington, and nearby CT communities. You will evaluate and treat older adults in homes or living facilities, delivering personalized PT programs in the patient environment.

Responsibilities include supervising assistants, coordinating with the rehab team, documenting care, and maintaining safe, functional therapy plans. Competitive benefits and relocation support offered.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy; or Master's degree in PT-related field in addition to a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of PT experience.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in-home or in living facilities for older adults.
• Supervise PT assistants and other personnel per state licensure.
• Adhere to treatment schedules, documentation, and meetings.
• Collaborate with rehabilitation team to tailor therapy plans.
