Genesis HealthCare seeks a licensed Physical Therapist to provide in-home outpatient rehabilitation, primarily in Vernon and surrounding CT areas. You will evaluate needs, design treatment plans, and deliver therapy to older adults in homes or community facilities.

The role emphasizes independence, caregiver education, and coordination with the rehab team. Qualifications include a PT license and a Master’s degree in Physical Therapy (or equivalent pathways) with relevant experience.

Qualifications

• Must hold valid Physical Therapy license in practice states.
• Master’s degree in Physical Therapy or equivalent, with alternative pathway options listed below.
• Bachelor’s degree in Physical Therapy with 5+ years of progressive PT experience.
• Foreign degree evaluated as US-equivalent where applicable.

Responsabilités

• Provide in-home outpatient physical therapy to older adults.
• Develop and deliver therapy plans to restore and maximize mobility.
• Educate patients and caregivers in home environments and community settings.
• Coordinate referrals to other disciplines as needed.
• Supervise PT assistants and trainees per state licensure.
